Potion Occupational Therapy  is committed to ensuring that each participant has knowledge of and access
to the provider’s complaints management and resolution system. Complaints and other feedback made by all
parties are welcomed, acknowledged, respected and well-managed. You are welcome to do so by filling out the above template, phoning or emailing the provider or doing so discretely by filling out a Feedback and Complaints form.

Further NDIS and Disability Advocate Feedback

If the Participant is not satisfied or does not want to talk to Potion Occupational Therapy, the Participant can contact the NDIS Commission by calling 1800 035 544 or visiting ndiscommission.gov.au for further information.

If the participant needs access to Disability Advocates in the complaints and feedback process, the participant can access advocates through the NDIS Commission recommended advocacy finder.
